Pennsylvania has reported a sixth measles-associated death this year as confirmed cases climbed to 1,467, the Pennsylvania Department of Health said on Wednesday, November 4, 2026, keeping the state at the epicentre of America's worst measles outbreak in decades. Hospitalisations reached 283, up from 265 in the October 28 update, when cases stood at 1,398 and five deaths had been reported. The new figures show an outbreak that began early in the year is still growing in the autumn, even as neighbouring states step up vaccination advice for infants most at risk.

Pennsylvania accounts for a substantial share of infections nationwide. The US Centers for Disease Control and Prevention (CDC) had confirmed 4,852 measles cases across the country as of October 29, more than double the 2,289 recorded in all of 2025. The gap between the state and federal death counts has itself become part of the story: the CDC has recorded only three measles-related deaths nationally, while Pennsylvania alone reports six, a difference Reuters attributes to a policy shift under Health Secretary Robert F. Kennedy Jr. to count deaths through National Center for Health Statistics mortality data rather than state health department figures.

Key Points

• Pennsylvania confirmed its sixth measles-associated death of 2026, with cases rising to 1,467 as of November 4.
• Hospitalisations in the state reached 283, up from 265 in the October 28 update.
• Two of Pennsylvania's six measles deaths have been in infants, health officials have said.
• Nationally, the CDC confirmed 4,852 cases as of October 29, more than double the 2025 total of 2,289.
• Wisconsin, with over 430 cases, urges early MMR vaccination for infants six months and older likely to be exposed.

What Happened

The Pennsylvania Department of Health released the updated figures on Wednesday, November 4, in its latest tally of an outbreak that has run through most of the year. The count of 1,467 confirmed cases is 69 higher than the 1,398 reported on October 28, and hospitalisations rose by 18 over the same period. The sixth death was reported alongside the new case total, although officials did not immediately release further details about the latest fatality in the Reuters report of the update.

The update is a follow-up, not a fresh outbreak: it revises the picture last published in late October, when Pennsylvania's toll stood at five state-reported deaths. Two of the state's deaths have been in infants, a fact that has sharpened concern among paediatricians, because babies too young for routine vaccination depend on the immunity of the people around them.

Background/Context

The 2026 measles resurgence is the largest the United States has seen in decades. After measles was declared eliminated in the US in 2000, imported cases occasionally sparked small clusters that were quickly contained by high vaccination coverage. That pattern broke down in 2026, when falling childhood vaccination rates and repeated introductions combined into sustained transmission.

By October 29, the CDC had confirmed 4,852 cases in 47 jurisdictions, mostly linked to outbreaks rather than isolated travel cases, with hospitalisation affecting roughly one in ten reported cases. Kentucky (204 cases) and Wisconsin (more than 430) show the same pattern of concentrated spread in under-vaccinated communities.

Pennsylvania Measles Outbreak: Why the State Remains the Epicentre

Pennsylvania's position at the centre of the national outbreak reflects both the size of its case count and the length of time transmission has continued. With 1,467 cases, the state alone accounts for roughly three in ten of the cases the CDC had confirmed nationally by late October, even allowing for the lag between state and federal reporting. Its hospitalisation total of 283 is also the largest single-state burden on hospitals in this outbreak.

The death-count discrepancy between the state and the CDC matters beyond statistics. State health departments typically count a death as measles-associated when the disease contributed to it, while the CDC's current approach relies on mortality coding through the National Center for Health Statistics. Reuters highlighted the gap because it leaves two official-looking totals that cannot both describe the same deaths in the same way.

Reactions and Official Guidance

The Pennsylvania Department of Health issued the updated figures through its regular reporting, and Reuters carried the new totals on November 4.

In Wisconsin, health officials moved on November 3 to urge early MMR vaccination for infants aged six months and older likely to be exposed, a step taken when an outbreak passes a threshold that puts babies at real risk. Infants normally receive their first MMR dose at 12 to 15 months, so an early dose is an outbreak measure, not a change to the routine schedule, and those children still need their standard doses later.

Why It Matters

A sixth death in a single state, from a disease preventable by a long-established vaccine, measures how far measles has re-established itself in the United States. Each death is a failure of the immunity shield that once kept the virus from circulating. When hospitalisations reach 283 in one state, the strain extends to emergency departments, paediatric wards and families forced to isolate while a child recovers.

What Happens Next

Health officials will continue to update Pennsylvania's case, hospitalisation and death totals as new reports arrive, and the next state update will show whether new cases are slowing or accelerating. Watch for a fresh county breakdown and further guidance on vaccination clinics.

The CDC's next update will test whether the country has passed the peak of the 2026 outbreak. For parents, the immediate step remains the simplest: check vaccination records, and ask a doctor about MMR vaccination if there is any doubt.

Frequently Asked Questions

How many measles cases has Pennsylvania reported in 2026?

As of November 4, 2026, Pennsylvania has reported 1,467 confirmed cases, 283 hospitalisations and six measles-associated deaths, according to the Pennsylvania Department of Health via Reuters.

How does Pennsylvania's death count differ from the CDC's national count?

Pennsylvania reports six measles-associated deaths under state criteria, while the CDC has recorded only three measles-related deaths nationally, a gap Reuters attributes to counting deaths through National Center for Health Statistics mortality data rather than state figures.

Why are infants at particular risk from measles?

Two of Pennsylvania's six measles deaths have been in infants. Babies are normally first vaccinated at 12 to 15 months, so younger infants rely on the immunity of those around them, and complications fall hardest on young children.

What is Wisconsin advising for babies during its outbreak?

Wisconsin health officials on November 3 urged early MMR vaccination for infants aged six months and older likely to be exposed, as its outbreak passed 430 cases. Those children still need their routine doses at the standard ages.

How many measles cases has the US recorded in 2026?

The CDC had confirmed 4,852 cases nationally as of October 29, 2026, more than double the 2,289 cases recorded in all of 2025, with Pennsylvania the largest single-state contributor.
