VOCERYQ FDA Approval: First DNA Medicine to Cut Surgeries for Rare Airway Disease

VOCERYQ FDA Approval: First DNA Medicine to Cut Surgeries for Rare Airway Disease

The VOCERYQ FDA approval marks a first for American medicine: the United States Food and Drug Administration has granted accelerated approval to VOCERYQ, a plasmid DNA-based immunotherapy for adults with recurrent respiratory papillomatosis, a rare and debilitating disease in which wart-like growths keep returning to the airways and must repeatedly be cut away. INOVIO, the biotechnology company behind the treatment, announced the decision on Friday, October 30, 2026, and described VOCERYQ as the first DNA medicine ever approved by the FDA in the United States. The approval offers a nonsurgical option to patients whose lives have been shaped by operation after operation, and it gives INOVIO's DNA-medicine platform its first commercial validation after years of research.

Recurrent respiratory papillomatosis, known as RRP, is caused mainly by human papillomavirus types 6 and 11. The growths, called papillomas, are usually benign, but they can block the airway, threaten breathing and damage the voice. Surgery has long been the standard treatment, yet the papillomas often grow back, so patients return to the operating theatre again and again. For them, the new approval is not an abstract scientific milestone. It is a chance to need fewer operations, to protect their voices and to spend less of their lives recovering from procedures that carry their own risks.

Key Points

  • The FDA granted accelerated approval to VOCERYQ on October 30, 2026, for adults with recurrent respiratory papillomatosis.
  • VOCERYQ is a plasmid DNA-based immunotherapy targeting HPV-6 and HPV-11, delivered with INOVIO's CELLECTRA 5PSP device.
  • In the RRP-001 study, 7 of 26 patients in the efficacy group (27 percent) needed no surgery in the 12 months after treatment.
  • INOVIO expects VOCERYQ to be available to order and ship in about 60 days, with full commercial launch in the first quarter of 2027.
  • Continued approval depends on a confirmatory study; the FDA requires a draft protocol by the end of 2026.

What Happened

The FDA granted accelerated approval for VOCERYQ (doruxapapogene ralaplasmid and lalikinogene sifuplasmid-gxfd), previously known as INO-3107, to reduce the frequency of surgical papilloma excisions in adults with recurrent respiratory papillomatosis. The indication was approved under the FDA's accelerated approval programme, which allows earlier access for serious conditions on the basis of an intermediate endpoint reasonably likely to predict clinical benefit. In this case, that endpoint was complete response, defined as no requirement for surgical intervention in the 12 months after treatment in the efficacy population.

INOVIO said VOCERYQ was approved in combination with CELLECTRA 5PSP, its proprietary DNA-medicine delivery device. The treatment is given by intramuscular injection using that device, which delivers electroporation — a brief electrical pulse that helps the body's cells take up the DNA plasmids. The plasmids encode antigens from HPV-6 and HPV-11, together with interleukin-12, prompting an immune response against the root cause of the disease rather than simply removing each growth as it appears.

The company said it expects VOCERYQ to be available to order and ship in approximately 60 days, and it anticipates full field deployment during the first quarter of 2027. INOVIO has engaged Syneos Health to support the recruitment and deployment of its medical and commercial field teams. A conference call was scheduled for Monday, November 2, 2026, at 8:00 a.m. Eastern Time, giving investors and clinicians a first chance to question the company about pricing, access and the confirmatory study.

Background: What RRP Does to Patients

RRP is rare, but its burden is heavy. The disease is characterised by small, wart-like growths in the respiratory tract. While the papillomas are generally benign, they can cause severe, life-threatening airway obstruction and respiratory complications. They can also affect the voice box, limiting a patient's ability to speak effectively, and repeated operations carry the risk of permanent damage to the vocal cords.

INOVIO's market research with patients and healthcare professionals indicates that a reduction of even one surgery matters, because every operation poses a risk of vocal-cord injury and brings costs in quality of life and money. Based on ongoing analysis of claims data, INOVIO believes there is a prevalent population of at least 25,000 patients with RRP in the United States, with additional patients diagnosed each year. That figure frames both the unmet need and the commercial opportunity: a small population, but one whose treatment has barely changed in decades.

The treatment pathway matters as much as the science. Following a standard-of-care surgical debulking of visible papilloma before the first dose, no additional surgical intervention is required as part of the nine-week dosing regimen. That sequence — one clearing operation, then a course of immunotherapy — is designed to shift care away from a cycle in which surgery is both the treatment and the recurring event.

VOCERYQ FDA Approval: The Trial Evidence

The approval rests on INOVIO's RRP-001 study, an open-label, multicentre trial in 32 adult patients with RRP who had undergone two or more surgeries in the year before treatment. During the VOCERYQ dosing window, no additional papilloma surgical excisions were required under the protocol. Of the 32 patients, 26 had required three or more surgeries for papilloma excision in the 12 months before treatment and were included in the efficacy evaluation. In that group, the median number of surgical excisions in the 12 months before the first dose was 4.5, ranging from 3 to 8.

Seven of those 26 patients, or 27 percent, demonstrated a complete response — requiring no surgical excision of papillomas after 12 months of follow-up. The company notes that such a result is not expected as part of the natural history of untreated disease. The complete clinical data were published in Nature Communications and in The Laryngoscope, the leading journal for the specialists who treat RRP, giving clinicians peer-reviewed detail behind the headline numbers.

The FDA had previously granted VOCERYQ both Orphan Drug and Breakthrough Therapy designations, and the European Commission granted INO-3107 Orphan Drug designation. Those designations reflect the rarity of RRP and the agency's view that the treatment could offer a substantial improvement over existing care. They do not, however, remove the central condition of accelerated approval: the benefit must still be confirmed.

On safety, VOCERYQ was found to be well tolerated. The most common adverse reactions, occurring in more than 5 percent of patients, included injection-site reactions, headache, fatigue, hypertension, nausea and dizziness. Most treatment-related adverse reactions were grade 1 and 2 and transient, and there were no grade 4 or 5 common adverse events. All 32 patients completed the study. The prescribing information warns that injection-site reactions should be monitored for at least 30 minutes after treatment, and that electroporation-related effects can include muscle contractions, transient bleeding and vasovagal reactions.

Reactions and Quotes

Dr Jacqueline Shea, INOVIO's president and chief executive officer, said: "With the approval of VOCERYQ, we believe our DNA medicine platform has the potential to transform the treatment paradigm for RRP by offering a patient-centric treatment option. VOCERYQ is designed to deliver what we understand matters most to patients, which is the reduction of the surgical burden caused by RRP." She thanked patients, investigators and the RRP Foundation, and said the company believes VOCERYQ has the potential to become a new standard of care.

Kim McClellan, president of the RRP Foundation, said: "This new FDA-approved therapy brings us an important step closer to ensuring every RRP patient has access to a non-surgical treatment option that works for them. In reaching this milestone, we have shown that patient voices matter in the drug development process." She added that patients have often had to endure repeated surgeries, and that the new option has the potential to further transform the treatment landscape for RRP patients.

Dr Simon Best, associate professor of otolaryngology at Johns Hopkins University and a lead investigator for the RRP-001 study, said: "The approval of VOCERYQ is an important step in improving the quality of life for RRP patients as every surgery carries the risk of laryngeal injury. I believe it is in the best interest of RRP patients that the standard of care quickly transition away from surgical interventions to immunotherapy after RRP is diagnosed." Karim Mikhail, director of the FDA's Center for Biologics Evaluation and Research, said patients could see "fewer surgeries, fewer disruptions, and more time living their lives on their own terms."

Why It Matters

The significance of this approval reaches beyond one rare disease. DNA medicines have been discussed for decades as a way to turn the body's own cells into producers of disease-fighting proteins, without the chemical adjuvants or lipid nanoparticles used by other platforms and without the anti-vector response historically seen with viral vectors. Until now, no DNA medicine had crossed the FDA finish line in the United States. VOCERYQ's approval therefore validates a platform, not just a product, and INOVIO's pipeline — focused on HPV-related diseases, cancer and infectious diseases — now has a regulatory precedent to point to.

For patients, the practical question is narrower and more urgent: how many operations can this treatment actually prevent, and for whom? A 27 percent complete-response rate in a small, open-label study is encouraging, but it also means most patients in the efficacy group still needed surgery in the year after treatment. The confirmatory study will need to show whether the benefit holds in a larger group and whether it lasts. Until then, clinicians will weigh VOCERYQ against surgery on a patient-by-patient basis, and payers will scrutinise a new therapy for a rare disease with a small but severely affected population.

The approval also lands in a competitive context. Another RRP therapy, Papzimeos, was approved in August 2025 and had already generated tens of millions of dollars in quarterly revenue by mid-2026. VOCERYQ enters a market that is no longer empty, and its case will rest on its surgery-reduction data, its delivery device and its launch execution in early 2027. For a company the size of INOVIO, that execution — field teams, patient access and clinician education — will matter as much as the science.

What Happens Next

Three dates now shape the story. First, the November 2 conference call, where INOVIO is expected to discuss launch plans and answer questions about the approval. Second, the roughly 60-day window before VOCERYQ is available to order and ship, followed by full commercial deployment in the first quarter of 2027. Third, the confirmatory-study timetable: the FDA requires INOVIO to submit a draft protocol by the end of 2026, with the aim of agreeing a final protocol by mid-2027.

Clinicians will also watch for real-world evidence once the treatment reaches patients outside a trial: how the nine-week regimen works in practice, how injection-site and electroporation effects are managed, and whether surgery rates fall as sharply as the trial suggests. Any safety signal, pricing dispute or access bottleneck will be reported quickly in a rare-disease community that is tightly connected through the RRP Foundation and specialist centres.
